Instructions for Registration with the North Warren Jr. Lacrosse Club:

1. Contact the Club Secretary for a US Lacrosse League Registration form.

2. Fill out the top section completely (do not worry about the Member ID#).

3. Fill out the left side of the form (do not worry about the shorts, jersey and position fields).

4. Skip the Coach section.

5. Provide the insurance info on the right and do not forget to sign and date the form.

6. Make check payable to North Warren Jr. Lacrosse. Registration fee is $xxx Maximum per family (2 or more players) $xxx.

7. Keep the bottom copy of the US Lacrosse form for your records.

8. Mail check and form to:
